I certify the reserve request I have submitted for inclusion in the Criss Library Reserve Collection are materials that are either required or supplemental reading for the course and semester indicated on the form. I understand that requests accompanied by course syllabi, rather than this form, are given a higher priority for processing and if my request requires copyright clearance payment I will be contacted to submit a course syllabus.
